The purpose of this registration form is to establish that you, Parent/Guardian, consent to your child receiving First Holy Communion in the coming year.
By completing this registration form, you enable the parish team register your child for reception of the sacrament and with your cooperation, prepare him/her for this important event in his/her faith life.
